Care Manager (RN)
Immediate need for a talented Care Manager (RN). This is a 04+months contract opportunity with long-term potential and is located in Columbia, SC(Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $37 - $39/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Outreaches to members after discharge from hospitalizations within 10 business days of discharge.
- Transition of Care Case Management for Medicaid members, telephonic outreaches to facilities and members.
- Assist facility staff with discharge planning efforts for members.
Key Requirements and Technology Experience:
- Key Skills; Case management
- Registered Nurse
- Discharge planning
- Case Management
- 2+years of nursing experience
- Case management experienced preferred, not required.
- Great computer skills and knowledge of navigating the systems
- Committment
- RN license (SC)
- BSN preferred but not required
- MS office – Outlook, Word, Excel, Teams
