Care Management Support Coordinator

Pay Range: $20 - $22/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities: 

• Shift Schedule: 8:00 AM to 5 PM PST Monday to Friday
• CM Support Coordinators are assigned at least 30 members daily to outreach, CM Support Coordinator will do 1 call attempt per day to #s listed for member
• If a # is invalid PC will also make a call attempt to get a valid #
• If member is reached, CM Support Coordinator will review CareGaps with members, offer to assist with scheduling appointment, complete HRA if needed, and confirm member has no other questions/needs
• Performance Metrics:
• 25 new members per day, 125 per week, and 500 per month
• Audit score of 90% or better
• Complete all outreach attempts within the 10-day maximum timeframe
• Use proper note documentation
• 80-100 Calls per day

 
Key Requirements and Technology Experience:  

• Key skills; 2 yr health care experience
• At least 1.75yr Experience with HIPAA procedures
• High volume inbound/outbound call experience
• Customer service
• High School Diploma
