Senior LTSS Service Care Manager (RN)

Pay Range: $38 - $44/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location) + Mileage reimbursement.
 
Key Responsibilities: 

• Will conduct in home assessments 3-4 times a week to put in LTSS services.
• Shift Time & Working Days: 8am-5pm for scheduled visits, may need to work after 5pm to complete documentation. No overtime.
• Day to day responsibilities of this the role and a description of the project (Outside of Workday JD):
• Will conduct in home assessments 3-4 times a week to put in LTSS services
• Describe the performance expectations/metrics for this individual and their team:
• Audit scores and Turnaround times.
• Tell me about what their first day looks like:
• Getting access to computer and assigned trainings

Key Requirements and Technology Experience: 

• Key skills; Home health, RN, Critical Care, Bedside Nursing
• Internal/External Groups with which the Candidate will interface:    
• Required Skills/Experience: RN    
• Preferred Skills/ Experience: Critical care or bedside care   
• Critical care or bedside care     
• Education Requirement: RN     
• Required Certifications:  RN
