Key Responsibilities:
- NOTE: Individual will work from their home and will also be required to go into the field. Field visits could include home visits, visiting member in group home/hospital setting, or attending community/agency meetings.
- This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
- The caseload is 100 members.
- This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
- Compliance with care management policies/procedures including outreach and documentation expectations.
- Individuals will be audited monthly to ensure charts comply.
- Key Skills: Pediatric case management experience.
- Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
- Health plan experience.
- Foster care and/or adoption experience.
- Bachelor’s Degree is required.
- Microsoft Office- Outlook, Word, and Excel.
