Registered Nurse-Case Managers

Pay Range: $80 - $85/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Our client is seeking a dedicated RN Case Manager to coordinate patient admissions, discharge planning, and utilization management in an acute care setting.
- This role involves working closely with a physician and multidisciplinary teams to ensure seamless transitions for patients, making recommendations for alternative levels of care, and providing support to families and caregivers.
- You will play a key role in patient advocacy, ensuring high-quality care while maintaining cost-effective resource use
Key Requirements and Technology Experience:

- One should have Demonstrated ability to utilize/apply the general and specialized principles, practices, techniques and methods of Utilization review/management, discharge planning or case management. Working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.).
- Demonstrated ability to utilize written and verbal communication, interpersonal, critical thinking and problem-solving skills.
- Bachelor's degree in nursing or healthcare related field
- Clinical Assessment & Discharge Planning: Ability to evaluate patient needs and develop comprehensive discharge plans with an interdisciplinary approach.
- Utilization Review: Knowledge of resource management and alternative care recommendations for optimal patient outcomes.
- Regulatory Compliance: Familiarity with healthcare regulations (TJC, Medicare, Medi-Cal) and documentation standards.
