Key Responsibilities:
- Conduct comprehensive member outreach and assessments by completing all required monthly and annual calls, face-to-face or virtual visits, collecting assessments on time, and coordinating with parents, DME providers, and PDN agencies to review care plans and resolve any member-related issues.
- Ensure accurate and timely documentation by compiling monthly ECC notes, maintaining detailed records of member status and care updates, and adhering to documentation, compliance, and performance standards while managing complex family interactions with strong communication skills.
- Skills-2 years pediatric experience required.
- BSN, but ASN also acceptable.
- Computer savvy
