Key Responsibilities:
- Provides outreach to members via phone or home visits to engage members and discuss care plan/service plan including next steps, resources, questions or concerns related to recommended care, and ongoing education for the member throughout care/service, as appropriate.
- Coordinates care activities based on the care plan/service plan and works with healthcare and community providers and partners, and members/caregivers to accommodate changes or progress, as needed.
- Serves as support on various member and/or provider inquiries, requests, or concerns related to care plan/service plan.
- Communicates with care managers, practitioners, and others as needed to facilitate member services and to ensure continuity of care/service.
- May support performing service assessments/screenings for members and documenting the member’s care needs.
- Supports documenting and maintaining member records in accordance with state and regulatory requirements and distribution to providers as needed.
- Follows standards of practice and policies compliant with contractual requirements and regulatory guidelines and standards.
- Ability to identify needs and make referrals to Care Manager, community based organizations, and Disease Manager.
- Provide education on benefits and resources available.
- Performs other duties as assigned.
- Complies with all policies and standards.
- This person will be completing outbound quarterly calls to members to conduct follow up questions.
- Will be assigned cases per month.
- Must be willing to complete research – such as identifying member information, connecting with providers, etc.
- 3 attempts per members, (Day 1, Day 2, Day 3) – based on if contact has been completed or not, new hire would then connect with PCP to ensure they have the correct contact information.
- There may be some stretch assignments that may be aligned based on volume.
- Team expectation to conduct 50-75 calls per day.
- Will be required to submit a daily report outlining their production for the day (through Outlook).
- Skills-2+ years in healthcare / managed care background, High Volume Calling (Inbound / Outbound) and Independent Work Ethic – Understanding and Respecting the Remote Work Environment.
- Requires a High School diploma or GED.
- 2+ years in healthcare / managed care background
- Must be comfortable completing outbound / inbound calling
- Looking for someone who can handle HIGH VOLUME calling – doctors office, front desk does not truly apply
- General Technology experience - Excel, Outlook, Word, TEAMS.
- Bilingual (Spanish, or Creole,)
- Previous work from home experience.
- Multi-task being able to complete calls and takes notes simultaneously.
- Must understand that this role is remote but with that they must still present professional on calls (attire, personal appearance).
- This person will be dealing with confidential medical information – the team is really looking for someone who has an office environment established where they will have a productive work environment.
