Community Resource Coordinator II

Pay Range: $21 - $23/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

  • Note; Location: Hybrid Role (Los Angeles, CA) – (Remote Home Visits
  • Shift: Training - 8am-5pm PST (MON-FRI) – about 4-week course // Work Shift: 8am-5pm PST (MON-FRI) – No OT – flex 1 weekend day per month (half day) will be notified prior
  • The purpose of our team is to support members by addressing both their health needs and the social factors that affect their well-being. We connect members to resources, close care gaps, and provide education through home visits, phone outreach, and community events like health fairs. Ultimately, our goal is to improve health outcomes and help members feel supported in every aspect of their care
  • Key projects often focus on connecting members to essential resources, closing care gaps, and supporting initiatives like maternal and reproductive health, wellness outreach, and community education.
  • Connects members to community resources to provide necessary care resources in a cost-effective manner, including providing members with known community resources, and will be doing telephonic campaigns to close care gaps.
  • Monthly TruCare structured note review ensures documentation is timely and accurate, PCP appointment status, community resources provided, success stories and achieve a member outreach completion/ contact rate of at least 35%.
  • This role is unique because it blends flexibility and community engagement—they can work from home, conduct home visits with members, and participate in health fairs to connect with the community.”

Key Requirements and Technology Experience: 

  • Key skills; [“member outreach”, “care coordinator” , "Care Management" , "Community Resource" , "social services" , "community health worker" , "care gaps" , "appointment scheduling" , "bilingual"]
  • Requires a High School diploma or GED
  • 1 – 2 years of related healthcare experience (member advocate, community resources)
  • Driver License and active auto insurance
  • Knowledge of Community Resources & Social Services – understanding local programs (housing, food, transportation, health, etc.) and how to connect members to them
  • Case Documentation & Data Entry – accurately recording member interactions, follow-ups, and outcomes in systems.
  • Care Coordination & Navigation – ability to guide members through health care systems, appointments, and benefit programs.
  • Bilingual preferred (English/ Spanish)
  • Cultural Competence – respecting and effectively serving diverse populations
  • Problem-Solving & Resourcefulness – finding creative solutions for members’ unique situations
  • Empathy & Compassion – understanding member needs and building trust
