Clinical - Care Manager (RN)

Pay Range: $37 - $40/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Case management for children medical services (0-age 21)
• Tier 4 & 3 members 
• Completing telephonic outreach monthly or quarterly
• Home visits upon member enrollment, quarterly for tier 3 members. 
• Attending meetings, meetings for members, participation in regional meetings and activities that are required by the team. 
• Teams, Zoom is used with the team.

Key Requirements and Technology Experience: 

• Skills-RN, Case Manager, Pediatric/child/Children.
• Pediatric case management / pediatric nursing.
• Community Care.
• Medicaid experience (AHCA).
• Requires a Degree from an Accredited School of Nursing or a bachelor’s degree in nursing and 2 – 4 years of related experience.
• Excel, Word, Outlook, Teams, Power Point, Zoom.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
• Shift Schedule: Monday - Friday 8 AM – 5 PM CST.
