Key Responsibilities:
- Provide direct outpatient nursing care for advanced heart failure, LVAD, and palliative care patients
- Perform patient rooming, vital signs, physical assessments, and phlebotomy
- Administer medications and perform EKGs and 6-minute walk tests
- Conduct telephone and EMR (EPIC) triage
- Educate patients and families on disease management, medications, and self-care
- Coordinate care with physicians, nurse navigators, and social workers
- Delegate and supervise care provided by non-licensed staff
- Maintain accurate and timely clinical documentation
- Participate in quality improvement, shared governance, and evidence-based practice initiatives
- Support patient safety, compliance, and regulatory standards
- Skills-Active Registered Nurse (RN) license (Georgia or Compact)
- BLS certification (American Heart Association or equivalent)
- Experience with direct patient care (heart failure, cardiology, or palliative care preferred)
- Strong clinical assessment and critical-thinking skills
- Ability to work in an outpatient clinical setting
- Proficiency with EPIC / EMR documentation and inbox triage
- Strong patient education and care coordination skills
- Excellent communication and interdisciplinary collaboration
- Ability to function in a fast-paced clinical environment
- Strong time management and organizational skills
- Ability to respond calmly and effectively in urgent or complex clinical situations
- Associate Degree in Nursing or Nursing Diploma required
- Bachelor’s Degree in Nursing (BSN) preferred
- Active RN License (Single-State of Georgia or Compact)
- BLS (American Heart Association or American Red Cross)
- Minimum 1 years of direct patient care RN experience
- Ability to work night shift (for this specific role)
- Ability to deliver individualized care in a fast-paced acute care environment
- Commitment to Hospital care standards, safety policies, and code of conduct
