Pay Range: $33 - $59/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Provide direct outpatient nursing care for advanced heart failure, LVAD, and palliative care patients
• Perform patient rooming, vital signs, physical assessments, and phlebotomy
• Administer medications and perform EKGs and 6-minute walk tests
• Conduct telephone and EMR (EPIC) triage
• Educate patients and families on disease management, medications, and self-care
• Coordinate care with physicians, nurse navigators, and social workers
• Delegate and supervise care provided by non-licensed staff
• Maintain accurate and timely clinical documentation
• Participate in quality improvement, shared governance, and evidence-based practice initiatives
• Support patient safety, compliance, and regulatory standards

Key Requirements and Technology Experience:

• Skills-Active Registered Nurse (RN) license (Georgia or Compact)
• BLS certification (American Heart Association or equivalent)
• Experience with direct patient care (heart failure, cardiology, or palliative care preferred)
• Strong clinical assessment and critical-thinking skills
• Ability to work in an outpatient clinical setting
• Proficiency with EPIC / EMR documentation and inbox triage
• Strong patient education and care coordination skills
• Excellent communication and interdisciplinary collaboration
• Ability to function in a fast-paced clinical environment
• Strong time management and organizational skills
• Ability to respond calmly and effectively in urgent or complex clinical situations
• Associate Degree in Nursing or Nursing Diploma required
• Bachelor’s Degree in Nursing (BSN) preferred
• Active RN License (Single-State of Georgia or Compact)
• BLS (American Heart Association or American Red Cross)
• Minimum 1 years of direct patient care RN experience
• Ability to work night shift (for this specific role)
• Ability to deliver individualized care in a fast-paced acute care environment
• Commitment to Hospital care standards, safety policies, and code of conduct
