Pay Range: $80k - $120k/annum.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities: 

• This role is accountable for the overall day-to-day oversight and management of the case management program including coordination, supervision, and administrative oversight of the case management team.
• This role will function as an expert clinical practitioner, case management subject-matter expert, resource, advisor and leader for the members of the case management team.
• Supervises and monitors professional and support staff and ensures that effective care coordination and case management practices are consistent with hospital policies, and applicable regulations and guidelines.

Key Requirements and Technology Experience: 

• Associate’s Degree in Nursing or Bachelor’s Degree in Nursing OR Master’s Degree in Social Work.
• 3+ years’ experience in Acute-Care Case Management or Social Work.
• 2+ years supervisory experience.
