Key Responsibilities:
- Assessing healthcare needs.
- Referring to health plan providers.
- Educating on benefits and expanded benefits.
- Providing community resources to address social determinants of health.
- Manage a caseload of 100 members.
- Document outreach notes, assessments, and care plans in an electronic health record.
- Compliance with care management policies and documentation.
- Monthly audits to ensure compliance.
- Key Skills:Case management, Assessing healthcare, care management.
- Pediatric case management experience
- Experience connecting families/children to local resources
- Bachelor's degree
- Proficient in Microsoft Office (Outlook, Word, Excel)
- Experience in health plans
- Experience in foster care and/or adoption
- Dependency Case Manager
- Child Protective Investigator
- DCF Navigator
- Adoption Case Manager
- Targeted Case Manager
- Community Agency Worker
- Training lasts 4 weeks, no PTO during training
- Must reside in the region covered
- Travel: Must travel within the assigned region; rarely leave that area.
