Pay Range: $40 - $46.07/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• A Health Care Occupational and Case Manager is responsible for coordinating and managing the care of patients with complex medical conditions or disabilities.
• They work closely with patients, healthcare providers, and insurance companies to ensure patients receive the appropriate care and support to improve their health outcomes.

Key Requirements and Technology Experience:

• Key Skills:Case Management, RN, occupational healthcare .
• Assessing patients' medical, psychological, and social needs.
• Developing individualized care plans in collaboration with healthcare providers and patients.
• Coordinating healthcare services, including medical treatments, therapies, and consultations.
• Monitoring patients' progress and adjusting care plans as needed.
• Providing education and support to patients and their families.
• Advocating for patients' needs and rights within the healthcare system.
• Communicating with healthcare providers, insurance companies, and other stakeholders to ensure coordinated care.
• Evaluating the effectiveness of care plans and making recommendations for improvement.
• Documenting patient assessments, care plans, and interactions.
• Keeping abreast of healthcare regulations, policies, and best practices related to case management.
• Health Care Occupational and Case Managers should have a background in nursing, social work, or a related field, along with experience in case management.
• They should have strong communication, organizational, and problem-solving skills.
• A compassionate and empathetic approach to patient care is essential.
