Key Responsibilities:

• Deliver direct nursing services to terminally ill patients, managing physical symptoms and providing emotional, social, and spiritual support with compassion and sensitivity.
• Perform comprehensive patient assessments using the Nursing Process and develop personalized care plans focused on patient goals and comfort.
• Educate patients, caregivers, and families on disease progression, signs of impending death, and medication management, respecting cultural practices and anticipatory grief.
• Perform initial presentations of hospice services, explain palliative care, obtain consents, and coordinate the election of hospice benefits.
• Facilitate medication management, including assessment, administration, and coordination with pharmacy services to ensure maximum patient comfort.
• Supervise the work of CNAs and collaborate effectively with an interdisciplinary team (IDT) to ensure seamless care delivery.
• Uphold all clinical, regulatory, and safety standards, including documentation in the EMR and adherence to TJC Patient Safety Goals.
• Maintain a valid driver’s license and auto insurance for potential travel related to patient care coordination.

Key Requirements and Technology Experience:

• Must have skills: Registered Nurse (RN), Hospice/Palliative Care, BLS, Valid Driver's License & Auto Insurance
• Education: Associate's Degree in Nursing (ADN) or Nursing Diploma required; Bachelor of Science in Nursing (BSN) preferred.
• Licensure: Current RN License in Georgia or a Multi-State Compact License.
• BLS Certification (AHA or American Red Cross).
• Valid Georgia Driver's License (or valid state license).
• Proof of Private Vehicle Insurance.
• Experience: Minimum 2 years of acute care experience as an RN required. Prior hospice, palliative care, or oncology experience is highly valued.
• Skills: Exceptional interpersonal, collaborative, and customer service skills. Strong critical thinking, time management, and the ability to function with composure in emotionally sensitive situations. Basic computer proficiency.
