Key Responsibilities:
- CM Support Coordinators are assigned at least 30 members daily to outreach, CM Support Coordinator will do 1 call attempt per day to #s listed for member.
- If a # is invalid PC will also make a call attempt to get a valid #
- If member is reached, CM Support Coordinator will review CareGaps with members, offer to assist with scheduling appointment, complete HRA if needed, and confirm member has no other questions/needs.
- 25 new members per day, 125 per week, and 500 per month .
- Audit score of 90% or better .
- Complete all outreach attempts within the 10-day maximum timeframe .
- Use proper note documentation .
- 80-100 Calls per day .
- Key Skills: 2 yr health care experience .
- At least 1.75yr Experience with HIPAA procedures .
- High volume inbound/outbound call experience .
- Customer service .
- High School Diploma or equivalent GED .
