Care Manager

Pay Range: $34.50 - $41/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).Additional Mileage reimbursement.
 
Key Responsibilities:

• A CMS Care Manager is responsible for outreaching members on a scheduled frequency to assess their unresolved healthcare needs work through barriers or obstacles to those needs being met.
• This may involve outreach to providers, suppliers, vendors, or other internal Sunshine Health departments to coordinate the member’s care.
• CMS Care Managers are required to meet the contract requirements set forth by AHCA and DOH.
• At a minimum, means either monthly or quarterly phone contact with our members, completing annual needs assessment packages with them, and quarterly in-person visits with some of our members.
• Accurate documentation and outreach is monitored continuously by chart audits, caseload audits, and recorded call audits.

Key Requirements and Technology Experience:

• Skills-LMHC or LCSW, Pediatric, Home health or hospice.
• LMHCs or LCSWs who have at least 2 years of experience working with the pediatric population is required.
• In addition, we value experience working alongside the child welfare system, community mental health agencies, and trauma-informed practices.
• 2 years of experience working with the pediatric population is required.
• Child welfare system, community mental health agencies, and trauma-informed practices.
• Health Plan Experience.
• Requires a master’s degree in Behavioural Health or Social Work or a Degree from an Accredited School of Nursing and 2 – 4 years of related experience.    
• Software Skills Required: Microsoft Office – Outlook, Word, Excel
• Required Certifications: LMHCs or LCSWs.
• Bi-lingual.
• Child welfare system, community mental health agencies, and trauma-informed practices.
• Note-Remote [Hendry and glades. we would also look at a candidate in Immokalee specifically, which is technically collier county, but other areas of collier would not help us].
