Key Responsibilities:
- This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
- The caseload is 100 members.
- This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.
- Skills-Pediatric case management experience.
- Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.
- Bachelor’s degree
- Compliance with care management policies/procedures including outreach and documentation expectations.
- Individuals will be audited on a monthly basis to ensure charts are in compliance.
- WFH Field visits could include home visits, visiting members in group home/hospital setting, or attending community/agency meetings. (depending on employees’ location).
- Shift (Start/End Time): 8am-5pm, Monday thru Friday
Weekend work may be needed during Hurricane Season for disaster outreach.
