- Patient & Family Education: Provide clear, compassionate explanations of hospice services, the palliative care model, Medicare/Medicaid benefits, and financial responsibilities.
- Help patients and families make informed decisions during a vulnerable time.
- Clinical Assessment & Admission: Perform thorough in-person nursing assessments in homes, assisted living facilities (ALFs), or skilled nursing facilities (SNFs) to determine hospice eligibility based on disease progression and prognosis.
- Coordination & Collaboration: Obtain verbal certification and initial orders from attending physicians and the hospice medical director.
- Coordinate the ordering of initial durable medical equipment (DME) and emergency medication kits.
- Plan of Care Development: Develop the initial, individualized hospice plan of care in collaboration with the patient and family.
- Determine visit frequencies, introduce the interdisciplinary team, and provide crucial emotional support regarding transition and anticipatory grief.
- Documentation & Compliance: Ensure all admission documentation is accurate and complete, including advance directives and code status.
- Adhere to all hospice regulations and agency policies.
- Key Skills;Current RN license (Georgia or Compact State)
- Current BLS certification (required upon hire)
- Valid GA Driver's License, personal vehicle, and auto insurance
- Minimum 2 years of acute care experience
- Hospice, palliative care, or home health experience
- Associate's Degree in Nursing or Diploma in Nursing is required.
- Bachelor of Science in Nursing (BSN) is strongly preferred.
- Licensure/Certification:
- Current RN License in the State of Georgia or a Multi-State Compact License.
- Current BLS (Basic Life Support) certification required upon hire.
- Valid Georgia Driver's License, reliable personal vehicle, and valid auto insurance are required.
- Hospice and Palliative Care Certification (CHPN) is a plus.
- Minimum 2 years of acute care experience is required.
- Hospice, palliative care, or home health experience is highly preferred.
- Experience with complex patient/family education and goals-of-care discussions is essential.
- Exceptional interpersonal, communication, and empathetic skills.
- Strong clinical assessment, critical thinking, and independent judgment.
- Ability to work autonomously, manage time efficiently, and navigate emotionally sensitive situations.
- Proficiency with electronic medical records and mobile technology.
