Care Management Support Coordinator II

Pay Range: $20 - $22/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Shift: 8:00 AM – 5:00 PM PST (No Overtime)
- Make outbound calls daily to remind members about Care Gaps, i.e., preventative visits, and to complete the Health Risk Assessment if it is due.
- We reach out to Medicare members, mostly those who are over 65 or have Medicare due to health issues
- Assigned at least 30 members daily to outreach, SC will do 1 call attempt per day to the # listed for the member. If # is invalid, SC will also make a call attempt to PCP to attempt to get a valid #. If a member is reached, PCP will review Care Gaps with a member, go over the Health Risk Assessment if needed, offer to assist with scheduling an appointment, and confirm the member has no other questions/needs.
- SC will continue to call the member four more times, four different times of day, if the member is not reached on the first outreach.
- Performance indicators:
- 30 new members per day, 125 per week, and 600 per month
- Audit score of 90% or better
- Meet 10-day TAT
- Use proper note documentation
- 80-100 Calls per day

Key Requirements and Technology Experience:
- Key skills; Healthplan or Health/Medical industry
- High-volume call center
- Knowledge of Medicare/Medicaid, Medical Terminology, MS Excel/Outlook, & Customer Service
- 2+ years of RECENT Member Services in a Healthplan or Health-related industry. Must be comfortable with high-volume calls in a fast-paced environment.
- Knowledge of Medicare/Medicaid, Medical Terminology, MS Excel/Outlook, & Customer Service is REQUIRED
- Strong Typing Skills 40 WPM, Strong Computer and MS Office Skills. Multitasking/Interacting using multiple systems/programs simultaneously.
- Due to HIPAA, a reliable home internet connection (hard-wired) with a dedicated workspace is required.
- Proof of a High School Diploma/GED or higher is REQUIRED. Must have an on-hand copy of HSD/GED/Transcript.
- Attendance/Reliability is mandatory - no time off allowed first 90 days.
- Disqualifiers: Management has requested no CA candidates
