- Provide specialized nursing care to patients with advanced heart failure and palliative care needs
- Perform comprehensive patient assessments and monitor for changes in condition
- Develop, implement, and evaluate individualized care plans
- Administer medications, IV therapies, and cardiac-related treatments as prescribed
- Assist with procedures, admissions, transfers, and discharges
- Educate patients and families on disease management, symptom control, and end-of-life care options
- Collaborate with cardiologists, palliative care specialists, and interdisciplinary care teams
- Monitor and manage symptoms including pain, shortness of breath, and fatigue
- Maintain accurate and timely documentation in electronic medical records (EMR)
- Promote patient safety, comfort, and quality of life while supporting goals-of-care discussions and advance care planning
- Active Registered Nurse (RN) license (Georgia or Compact)
- BLS certification required; ACLS preferred
- Minimum 1 year of experience in cardiology, heart failure, palliative care, or acute care setting
- Strong clinical assessment and critical-thinking skills
- Excellent communication and interdisciplinary collaboration abilities
- Strong patient advocacy and compassionate care approach
- Bachelor of Science in Nursing (BSN)
- Experience in heart failure management or palliative care
- Specialty certifications such as PCCN, CHPN, or equivalent
