Pay Range: $33 - $58/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Patient Assessment & Planning
• Conduct psychosocial and functional assessments to identify care needs and discharge goals.
• Partner with the care team to create personalized, efficient, and safe transition plans.
• Evaluate chronic disease management needs and available community resources.
• Care Coordination & Collaboration
• Work closely with physicians, nurses, and utilization review staff to ensure smooth care progression.
• Facilitate interdisciplinary rounds and family conferences to align care plans.
• Anticipate and remove barriers that delay discharge or continuity of care.
• Care Progression & Documentation
• Monitor clinical care progression and escalate issues as needed to improve patient flow.
• Maintain accurate, timely documentation in the electronic medical record (EMR).
• Track avoidable days and report trends to improve patient outcomes and efficiency.
• Professional Growth & Mentorship
• Participate in continuing education, quality improvement, and leadership initiatives.
• Serve as a mentor or preceptor to interns or new nurses.
• Support department-based goals that enhance patient care and team success.

Key Requirements and Technology Experience:

• Key Skills;BLS, experience as a Registered Nurse
• Associate’s Degree in Nursing (Required)
• Bachelor’s or Master’s in Nursing (Preferred)
• Minimum 1 year of nursing experience in an acute care setting (Required)
• Active RN License (Georgia or Compact State) – Required
• Basic Life Support (BLS) – Required
• Strong assessment and care planning abilities
• Excellent communication and problem-solving skills
• Knowledge of local and state healthcare resources and discharge regulations
• Ability to work collaboratively in a fast-paced, multidisciplinary environment
