Key Responsibilities:
- Conduct comprehensive patient examinations, including history and systems review.
- Evaluate findings to determine clinical diagnoses for straightforward cases; seek guidance for complex cases.
- Develop patient-centered care plans considering family lifestyle, environment, and goals.
- Provide evidence-based interventions, including procedural care, wound care, trach care, and diabetic counseling, with mentorship as needed.
- Educate patients and families, identify adaptive equipment and orthotic needs, and ensure thorough documentation.
- Monitor outcomes to assess progress, adjust care plans, and support transitional services.
- Contribute to departmental policies, procedures, and program development.
- Maintain productivity, fiscal responsibility, and documentation accuracy.
- Participate in departmental planning meetings and initiatives.
- Key Skills: CLINICAL DOCUMENTATION, PT licensed, BLS
- Maintain current skills and knowledge through ongoing learning and departmental in-services.
- Support clinical education by mentoring students and serving as a credentialed Clinical Instructor (CI) when appropriate.
- Collaborate with physicians, healthcare team members, third-party participants, and families regarding patient goals and progress.
- Demonstrate professional judgment informed by standards of practice, evidence, practitioner experience, and patient values.
- Appropriately delegate tasks to PTAs, aides, and support staff.
- Participate in presentations, interdisciplinary rounds, and staff in-services to share knowledge.
- Apply current research and best practices to clinical care.
- Access policies, guidelines, and resources to support evidence-based practice.
- Participate in journal clubs and departmental research initiatives.
- Communicate evidence and rationale for treatment to patients and caregivers.
- Master’s degree in Physical Therapy required (Bachelor’s degree acceptable for licenses obtained prior to 12/31/2002).
- Physical Therapy License in PA and/or NJ (depending on work location).
- CPR Certification from a recognized hands-on, instructor-led program (e.g., AHA or Red Cross).
- Inpatient rehab experience required.
- Experience with neurologic/brain injury or higher-acuity patients required.
- Pediatric experience preferred but not required.
