Key Responsibilities:
- Serve as the key liaison for Alzheimer’s patients and their families
- Coordinate care across pre-op, acute, and post-care phases
- Educate, empower, and advocate for patients using evidence-based best practices
- Collaborate with a multidisciplinary team to enhance patient outcomes and care experience
- Lead performance improvement initiatives that make an impact
- Skills-BLS, experience as a Registered Nurse.
- BSN or MSN (required)
- Active RN License (Compact or GA) + BLS Certification
- 3+ years of adult acute care experience (bariatric, spine, or case management experience a plus)
- Strong communication, leadership, and organizational skills
