Care Management Support Coordinator II

Pay Range: $18 - $18.50/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Maintain logs and tracking for audit readiness and reporting.
- Performance Expectations: • Compliance: Maintain 100% adherence to contractual timeframes. •
- Quality: Achieve a 97% or higher quality rating on documentation handling. •
- Productivity: Complete 98%–100% of member service goals daily. •
- Error Rate: Maintain an average error rate of 3% or less. Interaction Level: • Communication is primarily virtual, using email, Teams, and shared systems. •
- Low interaction with external departments or cross-functional partners outside the immediate workflow. •
- High interaction within the team environment, including regular communication with team members, team leads, and the manager via Microsoft Teams for task updates, support, and collaboration. Work Environment: •
- Fully remote, structured, and production driven. • High focus on accuracy, timeliness, and compliance.

- As part of the Centralized Fax Team, the Care Management Support Coordinator II plays a critical role in ensuring that essential documentation is processed accurately and routed efficiently, helping prevent service delays and ensuring timely care for vulnerable populations
Key Requirements and Technology Experience:
- Key Skills;Organizational and multi-tasking ability – High
- Adaptability to change – High
- Prior MCO experience– Intermediate to advanced
- Proficiency in Microsoft Office and Teams
- Strong critical thinking and problem-solving skills
- Positive, team-oriented attitude
- Disqualifiers: • Inability to meet daily productivity or quality standards
- Lack of experience in structured, metric-driven environments
- Compliance: 100% adherence to contractual timeframes
- Quality: Maintain a 97% or higher quality rating
- Productivity: Complete 98%–100% of daily member service goals
- Error Rate: Maintain an average error rate of 3% or less
