Resource Specialist/Pediatric Case Manager

Pay Range: $24 - $26/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Individuals will work from their home and will also be required to go into the field.
• Field visits could include home visits, visiting member in group home/hospital setting, or attending community/agency meetings.
• 3 days infield – 2 days remote
• Pediatrics
• Case management
• Kids with trauma
• Training 4 weeks – shadow and reverse shadowing
• No PTO during training – 4 wks
• 3 days infield – 2 days remote
• Foster care and pediatric exp (Minimum 1 year , 2 years preferred .
• Monday – Friday 8-5 PM EST. Weekend work may be needed during Hurricane Season for disaster outreach.
• This position will be field based and will provide care management to assist with coordination of care, including assessing for health care needs, referring to health plan providers, providing education on benefits/expanded benefits, and provide community resources to address social determinants of health needs.
• The caseload is 100 members.
• This position will document in an electronic health record and will be required to document outreach notes, complete assessments, and complete a care plan on each member.

Key Requirements and Technology Experience:

• Key Skills :Case Management, Home visits, Children .
• Compliance with care management policies/procedures including outreach and documentation expectations.
• Individuals will be audited on a monthly basis to ensure charts are in compliance.
• 2 years Pediatric case management experience .
• Experience connecting families/children to local resources for various services including employment, housing, independent living, justice, foster care, etc.  
• Health plan experience .
