Key Responsibilities:
- Assessment & Planning-Assess patient medical, psychosocial, and care transition needs.
- Collaborate with physicians and care teams to determine the most appropriate discharge plan and care goals.
- Evaluate community, insurance, and support resources required for ongoing care.
- Care Progression-Monitor patient care progression to ensure timely and efficient treatment and discharge.
- Identify and resolve barriers to discharge, escalating when necessary for resolution.
- Participate in interdisciplinary rounds to align discharge goals and expected timelines.
- Disposition & Post-Acute Coordination-Develop individualized discharge plans in collaboration with patients, families, and the care team.
- Initiate referrals for post-acute services, including home health, rehab, DME, or skilled nursing placement.
- Ensure patient and family understanding of discharge instructions and follow-up care needs.
- Documentation & Communication-Document assessments, plans, interventions, and patient/family interactions accurately in the medical record.
- Communicate effectively with payer sources, utilization review teams, and community resources.
- Track and report avoidable delays or barriers impacting care progression.
- Mentorship & Professional Growth-Support new team members through orientation and mentorship when needed.
- Participate in departmental performance improvement and professional development activities.
- Maintain required competencies and education
- Skills-BSN, BLS, experience as a Registered Nurse.
- Associate Degree in Nursing or Diploma in Nursing required; Bachelor’s Degree in Nursing preferred.
- Active Registered Nurse license (State or Multi-State Compact).
- Basic Life Support (BLS) Certification required.
- ACM or CCM Certification preferred (not required).
- Minimum 3 years of acute care bedside RN experience required.
- Minimum 2 years of hospital-based case management or payer case management experience required.
- Strong communication, critical thinking, problem-solving, and coordination skills.
