Care Coordination RN Sr. PRN

Pay Range: $44 - $48/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Clinical & Transitional Care Coordination:
- Assess transitional care needs and develop comprehensive discharge plans.
- Collaborate with physicians, utilization review nurses, and interdisciplinary teams to support smooth care progression.
- Address psychosocial factors, resource needs, and social determinants of health affecting recovery and transition.
- Anticipate and resolve barriers to care and discharge efficiency.
- Communication & Documentation:
- Serve as a liaison among care teams, patients, and families.
- Document all assessments, interventions, and communications accurately and promptly.
- Track and report avoidable days and other performance metrics.
- Mentorship & Professional Leadership:
- Act as a clinical resource and mentor for newer nurses and staff.
- Support case reviews and teach best practices in transitional and discharge planning.
- Contribute to a collaborative, respectful, and learning-focused team culture.

Key Requirements and Technology Experience:
- Key Skills: BLS, experience as a Registered Nurse .
- Education: Bachelor’s Degree in Nursing (BSN required).
- Licensure: Active RN License (Single State or Multi-State Compact).
- Certifications: BLS required; ACM or CCM preferred.
- Minimum 3 years as a staff nurse in an acute care hospital.
- Minimum 2 years in hospital-based or payer-based case management.
- Strong communication, critical thinking, and care coordination skills.
