Care Coordinator
Immediate need for a talented Care Coordinator. This is a 06 months contract to Hire opportunity with long-term potential and is located in Florida (Remote). Please review the job description below and contact me ASAP if you are interested.

Pay Range: $24 - $25/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
Key Responsibilities:
- Shift Timings: 9 AM - 6 PM EST Mon to Fri
- Conduct outbound calls to members to discuss and follow up on care/service plans
- Coordinate care activities with healthcare providers, community partners, and members/caregivers
- Assist with member inquiries, concerns, and service-related requests
- Perform service assessments/screenings and document member needs as required
- Maintain member records in compliance with state and regulatory requirements
- Identify needs and make referrals to care managers, disease managers, or community resources
- Provide education on benefits and resources available to members
- Complete daily production reports and meet call volume expectations (50–75 calls per day)
- Ensure professional conduct in a remote work environment, with the ability to work on-site if needed (Tampa office location available)
Key Requirements and Technology Experience:
- Key Skills; Minimum 2 years of healthcare or managed care experience
- Comfortable with high-volume inbound/outbound calls
- General technology proficiency (Excel, Outlook, Word, Teams)
- Ability to multi-task, manage confidential information, and work independently in a remote environment
- High School Diploma or GED required
