Key Responsibilities:
- This team conducts post-discharge outreach to AZCH Members post hospital or facility release.
- Goal to ensure services/appointments are in place to prevent readmission.
- Team culture is positive, collaborative, and supportive.
- Key projects include helping to improve Plan All-Cause Readmission Rates and Follow Up After Hospitalization Rates.
- This is a temp role to support the team while key leader is out of office on LOA.
- In the morning, staff receive list of members needing post-hospital call.
- The staff is required to make 1st call and set out reminder task for 2nd call if unable to contact.
- If successful contact is made, staff will complete post hospital assessment, help close any identified gaps, make referral to CM if appropriate, provide education about self-management resources, and complete appropriate documentation.
- Any follow up calls for unable to reach members will be completed and mailing of UTR letter if needed.
- Staff are assigned daily calls however also have follow up call.
- We are reassessing the volume of calls in comparison to overall reach rate and other team priorities/needs.
- This target may increase/decrease depending on identified efficiencies.
- This role is unique that it carries no caseload and critical part of daily health plan operations.
- Key Skills; 2-4 years of related experience, medical or BH office setting, member/provide facing responsibility.
- Case Manager, Behavioral Health Case Manager, Medical Assistant, working with Medicaid/Medicare members, health insurance experience works well on this role.
- Health related experience (Medical or Behavioral He.alth) and Direct patient or client contact experience is required.
- Telephonic / communication / Computer skills
- Team player – someone willing to learn, has strong communication skills, etc.
