Pay Range: $71,000 - $85,000 annually (dependent upon experience).

Key Responsibilities:
- Patient Evaluation & Treatment: Perform thorough and concise patient evaluations to determine functional abilities and limitations.
- Develop and implement individualized treatment plans with consideration to the patient's physical, social, spiritual, cultural, educational, and age-specific needs .
- Clinical Documentation: Complete thorough and concise clinical documentation in a timely manner, including evaluations, progress notes, and discharge summaries. Enter charges accurately and maintain knowledge of reimbursement related to practice setting .
- Patient & Family Education: Provide education to patients, families, and staff regarding treatment plans, home exercise programs, and discharge planning.
- Communicate effectively with patients, families, departmental coworkers, and members of the medical care team .
- Care Coordination: Collaborate with physicians, nurses, case managers, and other healthcare professionals to ensure optimal patient outcomes.
- Participate in patient care rounds and case management conferences as appropriate .
- Supervision & Mentorship: Supervise ancillary staff, students, and volunteers as needed. Impart knowledge to develop the clinical skills of colleagues and participate in orientation of new team members
- Professional Development: Advance clinical skills through ongoing education, literature review, and implementation of evidence-based practice.
- Maintain awareness of system goals and align personal goals and work activities accordingly.
- Regulatory Compliance: Adhere to organizational, national, and state regulatory guidelines. Maintain professional licensure and required certifications .
- Skills-Current Physical Therapist (PT) license in Texas
- Current BLS certification (within 90 days of hire)
- New Graduates Accepted and Encouraged to Apply
- Strong patient evaluation and treatment planning skills
- Education: Master's or Bachelor's degree from an accredited Physical Therapy program.
- Experience: New graduates are welcome to apply! No prior experience required .
- Certification/Licensure:
- Current Physical Therapist (PT) license in Texas required
- Current BLS (Basic Life Support) certification required within 90 days of hire or transfer
- Skills: Must possess problem-solving skills necessary to accomplish job responsibilities.
- Strong communication skills with ability to relate professionally and positively to patients, family members, and facility staff .
- Exhibits proven face-to-face synergy and leadership skills with all levels of the healthcare team.
- Proficient computer knowledge with ability to operate multiple computer systems and standard rehabilitation equipment.
- High attention to detail in fast-paced and dynamic environment.
- Ability to multitask effectively while maintaining a high level of productivity.
