Pay Range: $38 - $42/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).
 
Key Responsibilities:

• Day-to-day responsibilities include: Making phone calls to parents of members to collect information to complete annual assessments, monthly required update call, a monthly face-to-face visit and if visit declined a second monthly call or a virtual visit replaces it.
• Calling DME companies and providers if a parent states a problem.
• Managing difficult parents at times, responses to escalations and/or complaints as needed. Monthly regulatory reporting compiling updates obtained on each member of their caseload.
• Expectations include: Complete all required calls/contacts/visits or calls in place of visits, collect assessments on time, compile monthly information regarding member status into an assessment called an ECC note that is required by end of month.
• Teams meeting or a phone call with the PDN agency every 6 months to review the care plan agency has provided.
• Great documentation needed, education provided.
• We have approachable and supportive leadership.
• The candidates are highly supported by the other teammates as well, as we are a close group with frequent interaction with one another.

Key Requirements and Technology Experience:

• Must have skills: - Case Management, Registered Nurse, Pediatrics, Field visits
• RN with associate’s degree
• 2 years Pediatric RN experience
• Field visits required
• Computer literacy.
